Patient-specific implants are gaining popularity in the management of orthognathic surgery. The application of PSI were initially focused on the management of complex dentofacial deformities. Success in these cases resulted in a movement to place custom fixation on cases that would be labeled as "routine" and straightforward. A deep review of case selection, surgical time, accuracy, comparison of costs, incidence of removal and controversies between PSI and stock fixation is needed. Industry supports the increased utilization of PSI and has positioned key opinion leaders to fuel the trend. Time will tell how hospitals and third-party providers view the escalating cost of custom fixation. This session will provide points and counterpoints in an effort to educate surgeons on the "true" indications for patient-specific implants.
