The relation between dentofacial deformities (DFD) and temporomandibular joint disorders (TMD) is complex. They may concur, precede or succeed each other. Either or both could be the chief complaint(s) of a patient presenting to an oral and maxillofacial surgeon, necessitating a multidisciplinary approach in management. Although the evidence of a causal relation between dentofacial deformity and TMD is weak, some patients may show improvement in TMD after orthognathic surgery while some without preoperative TMD can develop TMD after orthognathic surgery. Most OMSs are well-trained to manage dentofacial deformities with orthognathic surgery but frequently face a dilemma on approach to management of the concomitant TMD symptoms. Options for management of TMD are multiple, ranging from non-surgical modalities to arthroplasties with or without reconstruction. With novel diagnostic tests and currently available evidence-based literature data supporting management of TMD, these modalities can be administered in a staged or simultaneous manner. This session aims to educate practicing OMSs as well as trainees on current evidence-based management of TMD as per the AAOMS Special Committee on Temporomandibular Joint Care (SCTMJC) while comanaging dentofacial deformities. Case examples of various treatment options will be included.
Learning Objectives:
At the conclusion of this presentation, participants should be able to:
Understand pathophysiology and diagnosis of TMD in patients with dentofacial deformity.
Identify the algorithm for management of TMD in the presence of dentofacial deformity.
Describe various surgical and non-surgical treatment options for management of TMD in patients requiring orthognathic surgery.
Discuss indications of staged and/or concomitant surgical management of TMD and dentofacial deformity.
